Gr 11 Health Requirements

This is a crucial alert that all NLPS Juniors are mandated to have a Physical Exam within the LAST YEAR with the HEALTH ASSESSMENT RECORD COMPLETED and submitted to the school nurse before they can enter school. The first day of school is Tuesday, September 3.

Please call your child’s 
doctor and arrange for a physical to be completed. The Health Assessment Record must also be completed. This document must be returned to the school no later than Friday, August 30
 to ensure your child can start on the first day of school.  

Please also note the following: 

  • Due to the recent measles outbreak, CT Dept of Health and Education will NOT allow any extensions this year. Exams must be completed and records filed with school nurse, prior to September 3, no exceptions to this date. 
  • It is recommended that this exam be performed by your private physician who is more familiar with your child’s medical history. 
  • The required private physical examination form can be downloaded by clicking here. The doctor’s private examination may be done within one year from the start of the current school year. Once the private examination is completed, return the private physical form to your school nurse. 
  • Should you need to have the physical exam performed by School-based Health Clinic, an enrollment form must be completed and an appointment made. Please call 860-437-4555 ext. 329
